Got a new Phone Number or Email Address ?  Want to add or remove the name of an authorized buyer?  Need to submit your latest TPT license? NO PROBLEM !

Simply complete the form below, attach copies of any paperwork and hit the “Update” button and your new information will be added to your account.

Your Account Number IS REQUIRED for verification.
Your Account Number can be found at the top left of your invoice after the words “Sold To:” and right above the company name.

Photos / Scaned COPIES of Licenses should be attached as files using the “Upload Copies of Licenses” section at the bottom of the form.

Account Update Information

Please attach copies of your TPT (Arizona Customers) or Sales Tax License (Non-AZ) license along with a copy of the business owner's Driver's License or State ID and a copy of your EIN.
